Family Plan

Oklahoma State Department of Health Family Emergency Plan:

I communicate or someone in my family communicates using American Sign Language (ASL).

List the name of family members, parents, children, or guardian.

Make sure your family has a plan in case of an emergency. Before an emergency happens, sit down and decide how you will get in contact with each other, where you will go, and what you will do in an emergency. Keep a copy of this plan in your emergency supply kit or another safe place where you can access it in the event of a disaster.

Please list out of town contact name, their phone number, and email address. Put down your neighborhood meeting place and address. Put down out-of-neighborhood meeting place and address. Put down out-of-town meeting place and address.

Write down where your family spends the most time: work, school, and other places you frequent. All schools, daycare providers, workplaces, and apartment buildings should have site-specific emergency plans that you and your family need to know about.

Please list the evacuation location and address. List any medical equipment you may use, and serial number.

Other important information: Please list your doctor's name and telephone number. Please put down your pharmacist's name and telephone number. Please put down the name of your medical insurance, the telephone number, and policy number. Please put down your homeowners or rental insurance information, telephone number, and policy number.
